At TMEDY INC ("TMEDY," “we,” or “us”), we respect and are committed to protecting your privacy. Generally, you can browse through our website without giving us any information about yourself. This privacy policy is meant to help those that use our services to explore providers or book appointments (“users”) and doctors and other healthcare specialists, professionals, providers, organizations or agents, or affiliates thereof that use our platform services (“Healthcare providers,” and collectively with users, “you,” or your”) understand how we treat your personal information. When we do need to collect your Personal Identifiable Information (defined below) to provide you with the Service, or when you choose to provide us with your Personal Identifying Information, this Privacy Policy describes how we collect, use, and disclose your Personal Identifying Information, except for personal health information submitted by you in the course of using our Service, which is covered by our Notice of HIPAA Privacy Practices (the "HIPAA Policy"). Any conflict between this Privacy Policy and the HIPAA Policy with respect to such submitted personal health information shall be resolved in favor of the HIPAA Policy. Information Collection

TMEDY Inc. collects several types of Personal Information. For example, to use some of its services and access certain areas of the Site, you must provide Personal Information that can be traced back to You, such as full name, street address and zip code, telephone number, e-mail, gender, date of birth, insurance coverage, social history and known health characteristics and medical needs. We collect this Personal Information that You voluntarily provide through the use of online data entry, survey, product purchase, and registration forms. Tmedy gathers most of this Personal Information from You during an initial voluntary User registration process for the Site. Tmedy also occasionally asks you for Personal Information at other times, including when you complete optional survey forms, fill out optional contact forms for additional information on particular products and services, or use a particular service.

We collect information about your location when you use or access our site. The degree of precision of the location data varies depending on the source of such information. Those sources include. Your Internet browser and your IP address. We collect and use this location-related data in order to:

  • Deliver content that is relevant to you based on your location

  • Restrict certain access to our site.

To access and use TMEDY Inc On-Line Appointment Request ("OAR") or schedule an appointment using TMEDY Inc On-Line Appointment Schedule ("OAS") (individually and collectively referred to as the "Appointment Services") to make an appointment with providers of healthcare services (collectively, "Healthcare Providers"), You must submit to Us the requested appointment information (the "Appointment Materials") during the completion of the Appointment Services scheduling process within the Site.

This personally identifying information that You submit with the Appointment Materials can be traced back to You and will include or be linked or associated with health-related information, such as physical or mental health conditions and the treatment of these conditions and other health-related information that You choose to share with Us.

The combination of this Personal Information with health-related information provided by you with the Appointment Materials is called "Protected Health Information" and is subject to special rules under a federal law called the Health Insurance Portability and Accountability Act of 1996 or "HIPAA", for short. Tmedy Inc. treats all personally identifiable information received from you with the Appointment Materials or created from your use of the Appointment Services as Protected Health Information, regardless of whether an appointment is scheduled or a scheduled appointment is fulfilled. Personal Information that includes or is linked or associated with health-related information that You submit elsewhere on the Site without the Appointment Materials is not Protected Health Information and is not treated as such by Tmedy Inc. or under HIPAA.

The Privacy Policy's references to "HIPAA" are to this law and the various laws that have amended it, including most recently the Health Information Technology for Economic and Clinical Health Act.

Cookies and Tracking Technology

A "cookie" is a small data file that certain websites write to your hard drive when you visit them. A cookie file can contain information such as a user ID that the website uses to track the pages you've visited, but the only PII a cookie can contain is information you supply yourself. A cookie can't read data off your hard disk or read cookie files created by other websites. You understand that, by logging into the App on your mobile device, some information pertaining to your medical treatment may be stored on your mobile device. We are not responsible for any unauthorized access by any third party to such information on your mobile device.

Location Information. When you use our App, we may collect and store information about your location by converting your IP address into a rough geo-location or by accessing your mobile device’s GPS coordinates or course location if you enable location services on your device. We may use location information to improve and personalize our App for you. If you do not want us to collect location information, you may disable that feature on your mobile device.

Opt-Out

Opt-Out. We may periodically send you free newsletters and e-mails that directly promote our Service. When you receive such promotional communications from us, you will have the opportunity to "opt out" (either through your account or by following the unsubscribe instructions provided in the e-mail you receive).
